Teams Toolkit: Multi-root workspace support

The Teams Toolkit extension for VS Code doesn't support multi-root workspaces.

Support added for multi-root workspaces:

Describe alternatives you've considered

• Multiple VS Code windows for each folder with Teams Toolkit being used.
• First folder in multi-root workspace is the one that needs Teams Toolkit

Additional context
I had a search through this repo before raising and didn't find any issues/discussions indicating multi-root support had been implemented.
