Won't add new bans to website - Getting this error!

So before someone asks me to do this I have already done two clean installs of this version of sourcebans from scratch and followed the manual accordingly and am still getting the same problem as follows:

When I imported the bans to the site everything worked fine and dandy. When launching my CS:GO Server there is no errors in the logs UNTIL I use the admin menu to ban someone. It bans them from the server, yes but what it does not do is add the ban to the sourcebans page. After banning someone it then throws a sourceban error in there. The error I am getting is as follows:
