Getting no hero when random

Normally a player gets a random hero if he does not select one when the time runs out.
However, there's a small chance that he gets no hero for the whole game. His hero icon on top of the screen will be blank and the respawn time under it will always show "1" throughout the game.
This happened to one of my teammates who said it's the second time this happened to him.

Reproduce (happens randomly):

1. Game mode 10v10
2. Frantic
3. Wait till the time for selecting a hero runs out. See if you get a hero or not.
