Possible new fields
vaccine_strains [string] - The specific vaccine offered by the facility (H1N1 vs H3N2).

age_group [string]- The recommended age group for the specific vaccine.

vaccine_type [string] - Live attenduated (nasal spray), trivalent inactivated (injusted) and thermisal free version of the inactivated.

insurance [string] - Is insurance accepted at the facility?

Possible issues with fields
begin_date - Because a ‘begin’ and ‘end’ date and time is not suffice for facilities with inconsistent dates and times of operation, the spreadsheet might be more useful if there's separate fields for Days-of-the-Week and a time field associated with each day.

end_date - The information necessary to display SFDPH Flu shot data is 'Days' (Monday, etc) / 'Dates' (16th, 18th) / Time

begin_time - Because several city facilities have times that either vary by day-of-the-week or are closed for lunch hour, a system more sophisticated than 'begin time' / 'end time' is necessary.

Questions with fields
facility_id - would this be a city assigned id?

eligibility - how would this be defined?

notes - What type of information do you imagine here?

file_start_date - Could this be the location to put the whole schedule when it doesn't fit in the more rigid fields above.
