This is related to AtlasOfLivingAustralia/fieldcapture#3334

The MetadataController::excelOutputTemplate is using the deprecated activitiesModel() call to obtain the form definition which pre-dates the implementation of form versioning. As a result it can download an old template definition for a new form resulting in it not working.
